Steps for Perkeso HSP Screening

If you have received the ORIGINAL VOUCHERS (per the sample below) from your employers, you can proceed to the nearest PANEL CLINIC for your health screening. I have prepared a list of panel clinics by location for your easy reference. Hope it helps. 

This is a general guide. Some clinics have their own instructions but generally, you can use mine as a reference. I deal directly with the panel doctors and perkeso staff on a daily basis.

1. Make an appointment at the nearest panel clinic. I have saved your time by creating a list based on different locations.
2. Make a copy of the voucher and bring along to the clinic for voucher validation. If you’re a male, you only have 1 voucher. If you’re female you have 2 vouchers (the additional voucher has “mammogram” stated on it). The clinic will keep your original voucher for billing their billing purposes. 
3. If you need to proceed to a mammogram centre for screening, please ensure the clinic stamp (chop) on the copy voucher and bring it along to the centre.
4. You can proceed to any panel mammogram centre to do your mammogram screening. You can refer to the list I’ve created as well.
5. Normally it won’t take that long for the report to be given to you. It should be within the same day.
6. Once done, you may proceed to the same panel clinic for medical review.

For ladies, if you’re at Selangor area, you can perhaps proceed to Alpha Specialist Centre to do your mammogram screening. The mammogram screening over there is carried out by female radiographer.
